Creating the Optimal Clinical Environment

Nurse educators are responsible for both didactic and clinical education. Much thought and planning goes into determining clinical content and clinical placements.  Before you can negotiate clinical partnerships, you must decide on the type of clinical experiences students need and where they can get them.  After completing this week’s reading, and reflecting on your experiences as a student, consider how an educator should go about making these kinds of decisions. 

Initial Post: Use these prompts to think about the topic and share your ideas.

  • How would things like active learning, action and reflection, and higher level thinking influence your decisions about student placement (i.e. where would you place new students vs. senior students and why)?
  • Scheduling students on the same floor for longer periods of time allows students to adjust and feel comfortable in the learning environment.  On the other hand, more frequent changes may provide a greater variety of clinical experiences.  As an educator, how do you choose?  Which do you prefer? Which do you think students prefer? Support your answers with information about best practices.
  • What balance of clinical observation vs. hands on care is optimal and why?
  • Who should have the greater role in guiding and supervising students on a clinical floor, the instructor or staff nurses/preceptors? How might this be negotiated/configured.

Scenario – There is a new Board running the hospital, and they are aggressive about the hospital being the best in the State. They have decided that you are best suited to lead the nurses in the hospital so that they will initially get status. From there, they are planning on having the nurses lead the way by having every nurse be a leader in their field. Your knowledge of how to reach these milestones is the reason that you were selected to lead the nurses in the hospital.

Instructions:

  1. Read the scenario above, and then answer the following questions:
    1. What will the nurses need to do so that the hospital will become magnet status and be respected in the industry?
    2. What other certifications and awards, etc., will the nurses need to obtain so that the hospital will be named a magnet institution?
    3. What types of organizational designs and structures will need to happen for magnet status?

Organizational Planning/Planned Change

 

Choose one of the following questions to answer for this week’s discussion board. Make sure to repost the question you selected at the top of your posting.

  1. How do you typically respond to change? Do you embrace it? Seek it out? Accept it reluctantly? Avoid it at all cost? Is this behavior like that of your friends and that of your family? Has your behavior always fit this pattern, or has the pattern changed throughout your life? If so, what life events have altered how you view and respond to change?
  2. Try to remember a situation in your own life that involved unnecessary change. Why do you think that the change was unnecessary? What types of turmoil did it cause? Were there things a change agent could have done that would have increased unfreezing in this situation?

Purpose

The Topic Search Strategy paper is the first of three related assignments. The purpose of this initial paper is to briefly describe your search strategies when identifying two articles that pertain to an evidence-based practice topic of interest.

Course outcomes: This assignment enables the student to meet the following course outcomes.

1. Examine the sources of knowledge that contribute to professional nursing practice. (PO 7)

2. Apply research principles to the interpretation of the content of published research studies. (POs 4 and 8)

Preparing the assignment:

Follow these guidelines when completing this assignment. Speak with your faculty member if you have questions.

a. Paper should be 3-4 content pages in length. Title and reference pages are required but are excluded from 3-4 -page length.

2) The paper will include the following sections:

a. Clinical Question

· Describe the problem. What is the focus of your work?

· Explain the significance of the problem in terms of patient outcomes. What health outcomes result from your problem? Or what statistics document this is a problem? You may find support on websites for government or professional organizations.

· PICOT question in support of the group topic.

· State the purpose of your paper. What will your paper do or describe? This is like a problem

statement. “The purpose of this paper is to . . .”

b. Levels of Evidence

· Identify the type of question being asked (therapy, prognosis, meaning, etc.).

· What is the best type of evidence to answer that question (e.g., RCT, cohort study, qualitative study, etc.)?

c. Search Strategy

· List search terms and results.

· Databases used (start with the CU library). Link your search with the PICOT question described above.

· Refinement decisions. As you did your search, what decisions did you make in refinement to get your required articles down to a reasonable number for review? Were any limits used? If so, what?

· Identification of two (2) most relevant articles (primary sources published within the last 5 years).

d. Format

· Correct grammar and spelling.

· Include a title page.

· Use of headings for each section.

· Clinical Question, Levels of Evidence, Search Strategy, and Conclusion

· Adheres to current APA formatting and guidelines.

· 3-4 pages in length, excluding title and reference pages.
