This family story discussion has two parts:

Subjective
Danny Rivera is an 8 years old boy presenting with a wet cough. He reports that the cough has lasted three days. He describes the cough as wet and gurly.
Objective
The cough is temporarily treated by the children’s cough medicine his mother gave him. He reports that the cough is not aggravated by activity. He reports the cough gets worse at night, which keep him up at night. He reports tenderness of his throat. He reports a history of frequent rhinorrhea and cough. Risk factor includes second-hand smoke from father, history of pneumonia in the past year, and being overwiegt for his age.
Assessment
He has no acute respiratory distress, his lungs are clear to auscultation and is afebrile, He has current rhinorrhea, examination of the nose reveals boggy turbinate. His throat appears red with visible cobblestoning in the back. His respiratory rate is inscreased, and he present mild tachycardia.
Plan
I recomende calling his grandmother to pick him up from school. He should see his primary care provider within the next few days for an evaluation and tests to rule out asthma and allergies. In the meantime, he should be allowed to rest and should be given cough medicine as needed.

This assessment provides an opportunity for you to strengthen your understanding of how to plan and negotiate the coordination of care for a particular health care problem. Include physical, psychosocial, and cultural considerations for this health care problem. Identify and list available community resources for a safe and effective continuum of care.
NOTE: You are required to complete this assessment before Assessment 4.
Preparation- As you begin to prepare this assessment, you are encouraged to complete the Care Coordination Planning activity. Completion of this will provide useful practice, particularly for those of you who do not have care coordination experience in community settings. The information gained from completing this activity will help you succeed with the assessment. Completing formatives is also a way to demonstrate engagement.
Scenario
Imagine that you are a staff nurse in a community care center. Your facility has always had a dedicated case management staff that coordinated the patient plan of care, but recently, there were budget cuts and the case management staff has been relocated to the inpatient setting. Care coordination is essential to the success of effectively managing patients in the community setting, so you have been asked by your nurse manager to take on the role of care coordination. You are a bit unsure of the process, but you know you will do a good job because, as a nurse, you are familiar with difficult tasks. As you take on this expanded role, you will need to plan effectively in addressing the specific health concerns of community residents.
To prepare for this assessment, you may wish to:
· Review the assessment instructions and scoring guide to ensure that you understand the work you will be asked to complete.
· Allow plenty of time to plan your chosen health care concern.

Develop the Preliminary Care Coordination Plan
Complete the following:
· Identify a health concern as the focus of your care coordination plan. In your plan, please include physical, psychosocial, and cultural needs. Possible health concerns may include, but are not limited to:
· Stroke.
· Heart disease (high blood pressure, stroke, or heart failure).
· Home safety.
· Pulmonary disease (COPD or fibrotic lung disease).
· Orthopedic concerns (hip replacement or knee replacement).
· Cognitive impairment (Alzheimer's disease or dementia).
· Pain management.
· Mental health.
· Trauma.
· Identify available community resources for a safe and effective continuum of care.
Document Format and Length
· Your preliminary plan should be an APA scholarly paper, 3–4 pages in length.
· Remember to use active voice, this means being direct and writing concisely; as opposed to passive voice, which means writing with a tendency to wordiness.
· In your paper include possible community resources that can be used.
· Be sure to review the scoring guide to make sure all criteria are addressed in your paper.
· Study the subtle differences between basic, proficient, and distinguished.
Supporting Evidence
Cite at least two credible sources from peer-reviewed journals or professional industry publications that support your preliminary plan.

Additional requirements– Before submitting your assessment, proofread your preliminary care coordination plan and community resources list to minimize errors that could distract readers and make it more difficult for them to focus on the substance of your plan. Be sure to submit both documents.

The Nursing Legislation and Advocacy page on the FNA website identifies 4-different ways for you to get involved with advocacy. Discuss which way(s) you would most likely get involved with and why?
One of the four different ways to get involved with advocacy that I would get involved in according to the FNA website would be joining the Barbara Lumpkin Institute to have public policy education readily available to improve the healthcare system. By joining this group, I would be given the opportunity to annual symposium and provide financial support to nurses who are involved in healthcare policy (FNA). I love to enjoy researching different topics and interviewing my colleagues on how we can improve to be as safe as possible. I would emphasize the importance of patient-to-nurse ratios and how to manage the nursing shortage starting with staff burnout. There needs to be more support for bedside nursing as it seems like the rate of people going this route is less and less.
Find and peruse the Barbara Lumpkin Toolkit (BLT). What are your thoughts about the information provided in the toolkit and what did you learn that you didn’t know before?
I think that the Barbra Lumkin Toolkit (BLT) is an excellent tool to use for anyone looking to promote advocacy in the healthcare setting. Advocacy is such an important part of nursing not only for our patients but for ourselves as nurses as well. I especially liked learning about the “Ten Commandments for successful living with your legislator”. Something that I didn’t know before is that anyone can call or write to the legislators regarding their reason for change or alteration to current bills. I think that it is so important that nurses get their voices heard and share their opinions with the people who are involved with making changes to the healthcare system.

This is a continuation of the health promotion program proposal, part one, which you submitted previously.
Please approach this assignment as an opportunity to integrate instructor feedback from part I and expand on ideas adhering to the components of the MAP-IT strategy. Include necessary levels of detail you feel appropriate to assure stakeholder buy-in.

You have already completed the steps 1-4.
Do not resubmit part 1. Make sure you revise this initial submission according to your instructor’s comments.
To assist in maintaining harmony between Part I and 2 here you have a reminder of the previous paper outline:
· 1. Describe the health problem. Using data and statistics support your claim that the issue you selected is a problem. What specifically will you address in your proposed health promotion program? Be sure your proposed outcome is realistic and measurable.
· 2. Describe the vulnerable population and setting. What are the risk factors that make this a vulnerable population? Use evidence to support the risk factors you have identified.
· 3. Provide a review of literature from scholarly journals of evidence-based interventions that address the problem. After completing a library search related to effective interventions for your chosen health promotion activity, you will write a review that evaluates the strengths and weaknesses of all the sources you have found. You might consult research texts for information on how to write a review of the literature found in your search.
· 4. Select an appropriate health promotion/disease prevention theoretical framework or conceptual model that would best serve as the framework guiding the proposal. Provide rationale for your selection which includes discussion of the concepts of the selected model

· 5. Propose a health promotion program using an evidence-based intervention found in your literature search to address the problem in the selected population/setting. Include a thorough discussion of the specifics of this intervention which include resources necessary, those involved, and feasibility for a nurse in an advanced role.
· Be certain to include a timeline. (2 to 4 paragraphs- you may use bullets if appropriate).
· 6. Thoroughly describe the intended outcomes. Describe the outcomes in detail concurrent with the SMART goal approach.
The SMART goal statement should be no more than one sentence (1 paragraph).
· 7. Provide a detailed plan for evaluation for each outcome. (1 paragraph).
· 8. Thoroughly describe possible barriers/challenges to implementing the proposed project as well as strategies to address these barriers/challenges. (1 paragraph).
· Finish the paper with a conclusion paragraph (1 paragraph) without typing the word “conclusion” before the paragraph.

Remember, your Proposal must be a scholarly paper demonstrating graduate school level writing and critical analysis of existing nursing knowledge about health promotion.
