Explain the agonist-to-antagonist spectrum of action of  psychopharmacologic agents, including how partial and inverse agonist  functionality may impact the efficacy of psychopharmacologic treatments.

Neurotransmitters and some drugs that stimulate receptors are  called agonists. Agonists stimulate receptors to their fullest signal  transduction. Drugs that act as agonists are useful in patients who lack  or are deficient in agonist neurotransmitters. In situations where the  actions of a full agonist or partial agonists are undesirable, an  antagonist is required to revert the receptor back to the state where no  agonist exists, back to neutral. Partial agonists act as they sound,  and produce a conformational change of a receptor that is halfway  between the changes of a full agonist and the baseline conformation of  that receptor. An antagonist can also reverse the effects of inverse  agonists to the receptors baseline. An inverse agonist acts to change  the conformation of a receptor to completely inactivate it and remove  the baseline constitutive activity.

Compare and contrast the actions of g coupled proteins and ion gated channels.

Both G protein coupled receptors and Ion gated channels are a  form of signal transduction cascades in the brain. They are both  triggered by neurotransmitters, and many of the psychotropic drugs used  today affect one of these two cascades. They both perform when an  extracellular first messenger passes a message to an intracellular  second messenger. However, G protein’s second messenger is a chemical,  such as adenosine monophosphate, while the ion channel second messenger  can be an ion, such as calcium. There are two classes of ion channels,  Ligand gated and voltage sensitive ion channels. The ligand gated ion  channels act similar to g protein receptors because they use the agonist  spectrum. Voltage sensitive ion channels are opened and closed based on  the voltage charge across the membrane.

Explain how the role of epigenetics may contribute to pharmacologic action.

Epigenetics determines if inherited genes are expressed or not  expressed into proteins that make up the story of any one person. The  mechanism of epigenetics turns genes on or off by modifying chromatin.  These modifications are regulated by neurotransmitters, drugs and the  environment. So drugs introduced to patients can affect a person’s  epigenetics.

Explain how this information may impact the way you prescribe  medications to patients. Include a specific example of a situation or  case with a patient in which the psychiatric mental health nurse  practitioner must be aware of the medication’s action.

Understanding the way medications will affect patients based on  their genetic makeup, their current medications, the condition being  treated, what causes the imbalance, is all important when prescribing  medications. For example imipramine, a tricyclic antidepressant,  inhibits the reuptake of serotonin and norepinephrine, elevating these  neurotransmitters in the brain. It is important to understand if this is  the cause of the patients depression before prescribing this medication  because if incorrectly prescribed it could alter the patients  epigenetic mechanisms in an inverse way (Boks, et al. 2012).

Evidenced-based practice in healthcare utilizes the current best and most relevant research, clinical experience, and patient/family valued preferences to deliver healthcare that not only improves patient experiences, but also provides better quality care for the patient and empowers clinicians, which leads to increased job satisfaction (Melnyk & Fineout-Overholt, 2023). While research clearly shows the benefits of evidence-based practice, there are a multitude of barriers that stand in the way of it becoming the standard of care such as: the lack of knowledge pertaining to evidence-based practice, lack of belief that this will result in more positive outcomes, overwhelming patient loads, and resistance to change just to name a few. Overcoming these barriers is necessary to move from practices based on outdated research in order to be able to provide better and safer healthcare.

Utilizing evidenced-based practice within the scope of my nursing license promotes lifelong learning and continuously improved nursing practice. My hospital has recently implemented bedside shift report for nurses, which has been shown to improve patient satisfaction and safety. Bedside shift report allows me to visualize both the patient and the surroundings in addition to communicating the plan of care to the patient. Initially, there was much resistance to changing the process of our shift report. Some nurses reported overwhelming patient loads (e.g. “I have to give report to 4 different nurses”) as a reason for which they struggled to do bedside report. After much encouragement from peers and leaders, the transition to bedside shift report has improved nursing accountability and teamwork. Visualizing all of my patients first thing in the morning allows me to ensure accuracy of the report and prioritize care.

NUR 435 – Week 3 Discussion: Managing Change

tep 1 Consider a change in practice that took place at your workplace.

Step 2 Answer the Prompts:

·         What was the involvement of team members in the change process? Did “rank and file” members of the team participate on the team planning for change?

·         What steps were taken to prepare for change?

·         How were the goals and outcomes communicated to team members? Was the communication clear or could it have been improved?

·         What was the outcome of the change?

·         In your opinion, what elements are required in order to have a culture that is open to change?

This soft drink brand is recognized worldwide. The drink has one of the most recognized corporate logos and is sold in over 200 countries through 250 bottlers throughout the world.

This company’s story began in Atlanta, Georgia, in 1886, when pharmacist John Pemberton was experimenting with a recipe that he later mixed with carbonated water and began to sell at his drugstore. Two years after its invention, Pemberton sold his secret formula to a businessman by the name of Asa Candler, who formed a corporation to bottle and distribute the trendy drink. He later sold the rights to two other businessmen who wanted to bottle the drink to enhance distribution. Candler sold syrup that his company produced to these distributors, but not his secret formula. They simply mixed the syrup with carbonated water and bottled it. From there, the product became a household name as more people began to take the drink home to enjoy.

Competitors soon emerged. To ensure that people could tell the original from the competition, the distributors created the trademarked contour bottle in 1916 so that customers would recognize the original product. Candler ultimately sold his company in 1919 to a group of investors, with Robert Woodruff as the president. The new group wanted to make the drink available anytime and anyplace. To accomplish this, the new company started adding bottling plants all over the world.

The six Ps of the company vision statement highlight its purpose and desired future objectives—people, portfolio, partners, planet, profit, and productivity.

To assist in accomplishing its vision, the company established a foundation in 1984. This entity focuses on helping and giving back to communities worldwide. Some of the issues supported in various countries include water stewardship (providing safe, clean drinking water), lifestyle and behavioral change programs (e.g., nutrition, exercise, and behavior modification), recycling, and education.

STRATEGIC CULTURE

Assignment Overview

As a healthcare professional, you have many tools that you can use for various projects. In strategic planning and
delivery, strategic maps are often used to depict an organization’s or department’s strategy. In addition, they provide a
process that aligns organizational targets and initiatives with a defined mission/vision and desired outcomes.

Case Assignment

In the Module 1 SLP assignment, you identified the mission and vision statement of your strategic plan. For this
assignment, you are to take these statements and create a detailed strategic map. The strategic map will be included
in the final submission of your strategic plan in the Module 4 SLP.

You can find an example of strategy maps and templates at A Complete Strategy Map Template (Including Examples)
https://www.clearpointstrategy.com/complete-strategy-map-template-examples/

Your strategy map must cover at least 4 of the following categories:

1. Clients

2. Financials

3. Internal Actions/Perspective

4. External Actions/Perspective

5. Possibility of Growth and/or Learning

6. Processes
