There are many ways of knowing. Think about how you learn in both your personal and professional life. Discuss various ways you have gained knowledge through your observational experiences during your capstone practicum by answering the following questions:

  • What is the focus of your capstone practicum?
  • What have you observed thus far?
  • How do you determine if the information presented is valid and accurate?
  • Review the “hierarchy of evidence” model and determine where you seek most of your information for both your personal and professional life and why.
  • Provide personal examples.
  • Include at least 2 scholarly articles supporting your statements.

Discussion Post-Prevention of Shock

 

Your patient is a 42-year-old female that arrives in the ED with complaints of fever and not feeling well. She is currently undergoing chemotherapy for bladder cancer. She has an indwelling urinary catheter with scant amount of dark, foul smelling urine. She has a temperature of 102.2F, HR 136, BP 110/50 and RR 28. She is allergic to penicillin and Sulfa.

  • What type of shock is she experiencing?
  • What interventions do you anticipate the doctor will order?
  • What can you teach this patient about prevention of infection?
  • The doctor orders Bactrim. What should you be concern about? Why?

CASE STUDY: Active Labor: Susan Wong

Mrs. Wong, a first-time mother, is admitted to the birthing suite in early labor after spontaneous rupture of membranes at home. She is at 38 weeks of gestation with a history of abnormal alpha-fetoprotein levels at 16 weeks of pregnancy. She was scheduled for ultrasonography to visualize the fetus to rule out an open spinal defect or Down syndrome, but never followed through. Mrs. Wong and her husband disagreed about what to do (keep or terminate the pregnancy) if the ultrasonography indicated a spinal problem, so they felt they did not want this information.

Reflective Questions 

1. As the nurse, what priority data would you collect from this couple to help define relevant interventions to meet their needs?

2. How can you help this couple if they experience a negative outcome in the birthing suite? What are your personal views on terminating or continuing a pregnancy with a risk of a potential anomaly? What factors may influence your views? 

3. With the influence of the recent Human Genome Project and the possibility of predicting open spinal defects earlier in pregnancy, how will maternity care change in the future?
