Step 3: Respond to the following prompt (1 page, single-spaced, 12 point font)

1. Articulate who you think is responsible for the Rwandan genocide? Colonial powers (i.e., through institutionalized discrimination)? Those who gave the orders? Those who carried out the orders? Those in Rwanda who failed to stop it? Those outside Rwanda who failed to stop it? You may assert a single choice or assert multiple layers of responsibility. Either way, pinpoint why you assign responsibility in the way(s) you do.

2.  Next, apply the question of responsibility to how the “world” should respond to present-day, unfolding events. Describe your reaction to the events with the Uyghurs in China using the following 3 prompts:

(1) Describe your awareness or lack of awareness of Uyghur experiences in Xinjiang before this exercise, include consideration for the role of media coverage and global conversation when responding to threats of genocide

(2) Describe three details that stood out to you in the above materials that shape your understanding of Uyghur experiences and/or China\’s position (i.e., what do you know now)

(3) Suggest strategies for responding to current threats faced the Uyghurs. This is an opportunity to weigh-in with your opinion.

Assignment # 3: Culturally Sensitive Population Care Management Plan Paper ( pages excluding title page, references and appendix with list of organizations and links)

a.       Identify the physical or mental health disparity that this plan addresses: describe, using RECENT (not older than 5 years) peer-reviewed evidence, ONE specific physical or mental health disparity experienced by this population; three (3) causes of the health disparity, and what initiatives have taken place to address the health disparity (including what has worked and what has not worked). Describe what needs to be done now and in the next 12 months to address this disparity.

b.       Describe your selected aggregate with the physical or mental health disparity: including RECENT demographic information on the estimated number of individuals in the community served by the organization, the age range of this population, and a description of the environment where they live; provides 3 examples of cultural values and practices that impact the chosen population when a person seeks physical or mental healthcare services and follows the health care providers’ instructions; includes 3 information examples on this population’s perceived value of health and access to care; describe financial access issues including private pay for services. Describe the specific organization located in a specific community where you will implement your plan. Identify three evidence-based models of care that have been used with a similar population and physical or mental health disparity.

c.       Apply the cultural competency model to address how it will structure your plan in addressing the identified physical or mental health disparity.

d.       Identify strategies for eliminating physical or mental health disparity: describe 5 implementation strategies that are based on best practice evidence including adapting evidence-based models to reduce the identified disparity

e.       Proposed care management plan and implementation steps: describe the care management plan including the specific departments within the organization who will be involved.  Identify at least six (6) specific steps that need to be taken to implement this plan. Include who will be carrying out the specific implementation steps and the timeline for completing the steps.

f.         Organizations to consider for grant funding: provide three (3) names of organizations that you can apply for grant funding to support implementing your Population Care Management Plan; include information on the title of the grant being applied for and a link to the grant guidelines from each of the organizations in the appendix.

g.       Measuring projected health outcomes for the physical or mental health disparity addressed in the care management plan: describe three (3) specific measurable health outcomes to evaluate the plan’s success; describe who will collect, synthesize, create the outcomes report, and distribute the data. Determine and describe the specific tools that will be used to collect data, who will collect the data, and three (3) distribution channels that you will use to disseminate the findings. Consider cost of intervention and sustainability.

h.       Summary:  2-3 paragraphs summarizing each of the prior sections to use as an executive summary for the plan.

Looking at the 5-year trends for the population with health disparities being served by the identified program, are there any changes needed?

As a nurse-leader, how would you evaluate the chosen program?  What outcomes would you consider?  Project the sustainability of the program over the next 5 years.  Use population-based data such as health insurance records, Centers for Medicaid and Medicare, and CDC to analyze the prevalence of one health disparity targeted by the chosen program over a 5 year period using the latest data available.

  What study type is considered at the highest rung of the evidence-based pyramid? 

  The highest rung on the evidence-based pyramid is the meta-analysis tier. Meta-analysis is a group of systematic reviews of multiple studies to find and compare the absolute best evidence available for a specific condition, population, or intervention. There is a precise, systematic methodology to research, choose, review, and evaluate the studies to include them in the meta-analysis (Tracy, et al., 2023). It gains the top tier of the evidence-based pyramid because meta-analysis yields the strongest level of evidence after all these systematic reviews (Melnyk & Fineoout-Overholt, 2019). These specific meta-analysis studies then present a summary of the current best evidence available for the area of interest. The meta-analyses can then guide clinicians to make evidence-based decisions for the treatment of their patients.

1. Choose a topic that interests you.

2. If you are unsure what to advocate for or against, look at professional organizations for inspiration:

3. Look for advocacy groups that are working on the issue. We don't have to recreate the wheel.  See what strategies the advocacy groups are supporting. For example: Brady and Gifford non-profits focus on gun policy. 

4. Decide if you want to make change within an agency, at the local, state or federal level. Find the decision maker appropriate to that level. Click here to

5. If you are writing to a legislator, look at that person’s website to see their position on the issue. Please do not ‘preach to the choir’—that is, if they already support the issue, it does no good to throw more facts at them.

6. If you are addressing a national issue, and your legislator aligns with your proposed action, consider looking at the committee that would hear a bill about your issue.

Good Evening Professor,

 

              I work in one of the largest hospital systems in Atlanta. The facility I work for has the bed capacity for 686 patients. The hospital specializes in cardiovascular procedures (the implantation of the LVAD) and transplantation of liver, and kidneys. The hospital attracts people from across the country. People coming from literally every state, and sometimes from out of the country.

            The area where I see the EHR impeding patient care, and again this might be in part, due to end-user negligence, are providers not utilizing the information and data contained within the patient's EHR frequently enough.  We run off a system called EPIC (EHR). I have come to realize most hospitals in the state of Georgia, and other states use EPIC as well.  Embedded within the EPIC system is a hyperlink called Care Everywhere. The Care Everywhere tab allows all providers on the EPIC system to see notes, documentation, tests results, frequency of visits, and more. This data covers every hospital, clinical, urgent care, and office visit the patient has encountered so as long as the facility is on the EPIC system. I believe the shear amount of medical information becomes overwhelming and time consuming to comb through, especially for medical providers who are already seeing numerous patients at the bedside and in specialty offices. 

          Despite medical documentation being readily available to providers across the healthcare spectrum, there seems to be little time for a busy Cardiologist to review necessary documentation from the patient's Pulmonary doctor, or the notes placed from Endocrinology. Often a patient comes for a visit due to CAD, but the patient is also a diabetic with uncontrolled elevated blood glucose. We know one disease process feeds the other, but in spite of the EHR containing valuable patient data, providers are still missing imperative clinical information required to treat the patients holistically. 

          The one department which I see consistently using chart review to coordinate care with patients, providers, and families are RN Case Managers within the hospital setting.  RN Case Managers, and Care Coordinators are responsible for knowing what is missing in the patient continuum of care. The RNCM interfaces with the providers, and assists in providing education to both medical MDs and patients regarding critical clinical information which will advance the clinical process and bring better patient outcomes. McBride & Tietze (2018) discussed unintended harm, or consequences suffered by patients with the implementation of the EHR. One such consequence has to do with physicians required to research, and review copious amounts of previous documentation, as well as being required to spend greater amounts of time entering documentation.  Such time consuming tasks often create information, and important patient data not being read, or documented in an effort to save time, and so providers can physically see more patients.
