Health Promotion Proposal, Part 2
This is a continuation of the health promotion program proposal, part one, which you submitted previously.
Please approach this assignment as an opportunity to integrate instructor feedback from part I and expand on ideas adhering to the components of the MAP-IT strategy. Include necessary levels of detail you feel appropriate to assure stakeholder buy-in.
Directions
You have already completed the steps 1-4.
Do not resubmit part 1. Make sure you revise this initial submission according to your instructor’s comments.
To assist in maintaining harmony between Part I and 2 here you have a reminder of the previous paper outline:
· 1. Describe the health problem. Using data and statistics support your claim that the issue you selected is a problem. What specifically will you address in your proposed health promotion program? Be sure your proposed outcome is realistic and measurable.
· 2. Describe the vulnerable population and setting. What are the risk factors that make this a vulnerable population? Use evidence to support the risk factors you have identified.
· 3. Provide a review of literature from scholarly journals of evidence-based interventions that address the problem. After completing a library search related to effective interventions for your chosen health promotion activity, you will write a review that evaluates the strengths and weaknesses of all the sources you have found. You might consult research texts for information on how to write a review of the literature found in your search.
· 4. Select an appropriate health promotion/disease prevention theoretical framework or conceptual model that would best serve as the framework guiding the proposal. Provide rationale for your selection which includes discussion of the concepts of the selected model
For this assignment develop criteria 5-8 as detailed below:
You will submit just this section 5-8 as essay. Please do not resubmit Part 1.
Use a presentation page. Start the body of content with topic 5.
· 5. Propose a health promotion program using an evidence-based intervention found in your literature search to address the problem in the selected population/setting. Include a thorough discussion of the specifics of this intervention which include resources necessary, those involved, and feasibility for a nurse in an advanced role.
· Be certain to include a timeline. (2 to 4 paragraphs- you may use bullets if appropriate).
· 6. Thoroughly describe the intended outcomes. Describe the outcomes in detail concurrent with the SMART goal approach.
The SMART goal statement should be no more than one sentence (1 paragraph).
· 7. Provide a detailed plan for evaluation for each outcome. (1 paragraph).
· 8. Thoroughly describe possible barriers/challenges to implementing the proposed project as well as strategies to address these barriers/challenges. (1 paragraph).
· Finish the paper with a conclusion paragraph (1 paragraph) without typing the word “conclusion” before the paragraph.
Paper Requirements
Your assignment should be up to 3-5 pages (excluding title page and references).
Remember, your Proposal must be a scholarly paper demonstrating graduate school level writing and critical analysis of existing nursing knowledge about health promotion.
Finish the essay with a your reference page.
Please review the Grading Rubric for this Assignment.

Here’s a question I have for you this week. Compare and contrast the TAPS Health Assessment Tool and the CAPS Health Assessment Tool. Try creating a side-by-side chart so that at a quick glance you can identify the similarities and the differences. The differences are key as this is how to tell them apart.

A
S.M.A.R.T. goal is defined by its five key aspects or elements. Without all aspects, you might be goal setting, but not effectively creating a plan for success. Let’s take a closer look at the five elements of S.M.A.R.T. goals.
Specific
Specific goals have a desired outcome that is clearly understood. This might be a sales number or a product rollout goal. No matter what it is, the goal should be clearly articulated so that everyone is on the same page with the objective. Define what will be accomplished and the actions to be taken to accomplish the goal. Goal must be clearly defined —who and what
Measurable
These are the numbers used with the goal. You need to have a quantifiable objective so that you can track progress. Define what data will be used to measure the goal and set a method for collection. The success toward meeting the goal can be measured. Outcome must demonstrate levels of change or improvement.
Achievable
Goals need to be realistic in order to maintain the enthusiasm to try to achieve them. Setting lofty goals is good, but you may want to break them down into smaller, bite-sized chunks. If the goal is not doable, you may need to first ramp up resources to give yourself a shot at success. Ramping up resources would likely be its own S.M.A.R.T. goal. Goals are reasonable and can be achieved.
Relevant
Goals should be aligned with the mission of the company or specific project at hand. Don’t set goals just as an exercise for something to do. One way to determine if the goal is relevant is to define the key benefit to the organization or to your personal goal. The goals are aligned with current tasks and projects and focus in one defined area
Time-Bound
Goals should have a deadline. A goal without a deadline doesn’t do much. How can you identify success or failure? This is why S.M.A.R.T. goals set a final date. This doesn’t mean that all the work is done, but it means that you can evaluate the success of the endeavor and set new goals. Goals have a clearly defined time-frame including a target or deadline date.
Using the information presented as a guide create a SMART goal to improve the indicators of your health problem at short or long term [this is the same health problem you will be completing the PowerPoint on]. include a paragraph to introduce your topic and place it in context for your reader. Do not forget to cite your sources.
SMART goals help improve achievement and success. A SMART goal clarifies exactly what is expected and the measures used to determine if the goal is achieved and successfully completed.
Examples:
Not a SMART goal:
· Reach out to stakeholders.
Does not identify a measurement or time frame, nor identify why the improvement is needed or how it will be used.
SMART goal:
· The Department will launch communications with stakeholders by conducting three focus groups specific to needs assessment and funding by the end of the first quarter.
Please ensure you follow the SMART format.

How do we identify health Trends?
2
Answer with at least 6 sentences, one scholarly reference ( discussion doesn’t have to be long just enough to get the question answered )
Scenario 1: Myocardial Infarction
CC: “I woke up this morning at 6 a.m. with numbness in my left arm and pain in my chest. It feels tight right here (mid-sternal).” “My dad had a heart attack when he was 56-years-old and I am scared because I am 56-years-old.”
HPI: Patient is a 56-year-old Caucasian male who presents to Express Hospital Emergency Department with a chief complaint of chest pain that radiates down his left arm. He states this started this morning and has been getting worse, pointing to the mid-sternal area, “it feels like an elephant is sitting on my chest and having a hard time breathing”. He rates the pain as 9/10. Nothing has made the pain better or worse. He denies any previous episode of chest pain. Denies nausea, or lightheadedness. Nitroglycerin 0.4 mg tablet sublingual x 1 which decreased pain to 7/10.
Lipid panel reveals Total Cholesterol 424 mg/dl, high density lipoprotein (HDL) 26 mg/dl, Low Density Lipoprotein (LDL) 166 mg/dl, Triglycerides 702 mg/dl, Very Low-Density Lipoprotein (VLDL) 64 mg/dl
His diagnosis is an acute inferior wall myocardial infarction.
Question:
1. How does inflammation contribute to the development of atherosclerosis?

Purpose: The purpose of this assignment is to identify common patterns
of social behaviors within a specific social event or a social setting. Your
writing will illustrate your ability to interpret social interactions using
the content from units 1-4.
Activity: Conduct a participant observation procedure by observing a
group/social event. The event could be a trip to a theme park, enjoying a
play at a theater, or a dinner with family at a restaurant. It could also be
attending a religious gathering, hanging out with friends at a coffee
shop, attending a sports event like a baseball or a basketball game,
celebrating a cultural holiday, or attending a meeting at your workplace.
This is NOT a reflection of a past event. the observation of behaviors
must be conducted within 2 weeks of the deadline.
First, write a few sentences about the activity itself including what the
event was, how long it was, and why you chose it. Next, summarize your
observations and describe any behaviors that stand out. Discuss your
thoughts on the common social interaction patterns between the people.
Your writing should illustrate how the observation activity facilitated
your understanding of human behaviors. You should use 2-3 relevant
concepts from at least two different chapters from Units 1-4. The paper
should 2-3 completed pages, excluding the title and the references pages.
Use APA format throughout the paper including for the Title page, and
references and in-text citations
