Care Coordination Presentation to Colleagues

INSTRUCTIONS-Develop a 20-minute video presentation for nursing colleagues highlighting the fundamental principles of care coordination. Create a detailed narrative script for your presentation, approximately 4-5 pages in length. This is a video presentation.

INTRODUCTION- This assessment provides an opportunity for you to educate your peers on the care coordination process. The assessment also requires you to address change management issues. 

PREPARATION– You are encouraged to complete the Managing Change activity. Completing course activities before submitting your first attempt has been shown to make the difference between basic and proficient assessment.

Scenario

Your nurse manager has been observing your effectiveness as a care coordinator and recognizes the importance of educating other staff nurses in care coordination. Consequently, she has asked you to develop a presentation for your colleagues on care coordination basics. By providing them with basic information about the care coordination process, you will assist them in taking on an expanded role in helping to manage the care coordination process and improve patient outcomes in your community care center.

To prepare for this assessment, identify key factors nurses must consider to effectively participate in the care coordination process.

You may also wish to:

· Review the assessment instructions and scoring guide to ensure you understand the work you will be asked to complete.

· Allow plenty of time to rehearse your presentation.

Recording Equipment Setup and Testing

Check that your recording equipment and software are working properly and that you know how to record and upload your presentation. You may use Kaltura (recommended) or similar software for your audio recording. A reference page is required. However, no PowerPoint presentation is required for this assessment.

Include community resources, ethical issues, and policy issues that affect the coordination of care. To prepare, develop a detailed narrative script. The script will be submitted along with the video.

not required to deliver your presentation.

Presentation Format and Length

Create a detailed narrative script for your video presentation, approximately 4–5 pages in length. Include a reference list at the end of the script.

Supporting Evidence

Cite 3–5 credible sources from peer-reviewed journals or professional industry publications to support your video. Include your source citations on a references page appended to your narrative script. Explore the resources about effective presentations as you prepare your assessment.

The requirements outlined below correspond to the grading criteria in the Care Coordination Presentation to Colleagues Scoring Guide, so be sure to address each point. Read the performance-level descriptions for each criterion to see how your work will be assessed.

· Outline effective strategies for collaborating with patients and their families to achieve desired health outcomes.

· Provide, for example, drug-specific educational interventions, cultural competence strategies.

· Include evidence that you have to support your selected strategies.

· Identify the aspects of change management that directly affect elements of the patient experience essential to the provision of high-quality, patient-centered care.

· Explain the rationale for coordinated care plans based on ethical decision making.

· Consider the reasonable implications and consequences of an ethical approach to care and any underlying assumptions that may influence decision making.

· Identify the potential impact of specific health care policy provisions on outcomes and patient experiences.

· What are the logical implications and consequences of relevant policy provisions?

· What evidence do you have to support your conclusions?

· Raise awareness of the nurse's vital role in the coordination and continuum of care in a video-recorded presentation.

· Fine tune the presentation to your audience.

· Stay focused on key issues of import with respect to the effects of resources, ethics, and policy on the provision of high-quality, patient-centered care.

· Adhere to presentation best practices.

Statistics play an important role in the management of health care organizations and in decision making and strategic planning. Collecting, analyzing, and utilizing data appropriately impacts financial management of the organization and the quality of patient care delivered. Decisions in clinical medicine are driven by evidence-based practice, as are those in health care management. A health care manager must understand and use evidence in making informed decisions that improve the operations and financial status of the organization as well as deliver quality of care accessible to those who need it and with consideration of the costs involved.

In this assignment, you will choose a utilization review statistic from the following list and review a research  or government publication, as outlined below.

Choose a health care statistic that health care managers would use, such as one of the following:

• Hospital-acquired condition (HAC)
• Emergency department visits
• Labor and delivery suite usage
• Ambulatory surgery procedures
• Hospital death rates
• Cesarean-section rate
• Wait time
• Adverse drug events

Research your chosen statistic, and provide information from an outside source—such as a journal article, accreditation agency report, government site, and so forth—that discusses the health care statistic in a real-life setting or situation.

 in APA format that discusses the following:

• What was the reason for the research that was performed or for the data being collected?
• What was the source of the data presented?
• What types of descriptive statistics and graphical representations of data were used?
• What were the research question(s) and the significant findings of the article?
• How could the information presented be used to inform decisions or improvements?

Cite references using APA format, and remember to include a citation for the article that you are describing.

Reflection # 7

Often, after a class is done for the week, we drop it and move on. The goal of reflection assignments is to encourage you to reflect on what we did in class that week, after the class to consolidate your learning. There will be specific prompts for you to answer; sometimes this is a reflective assignment, and other times it is more of an application assignment (e.g., given a case example and having to apply something from class). The hope is that this will help you to use what you have learned to ensure concepts “stick” or to think critically about your response to what was learned.

1. What are some factors that may have maintained Saiku's PTSD symptoms?

2. How do you think this trauma (and subsequent symptoms) impacted this child's beliefs about himself, other people, or the world in general?

3. Based on the clip, what are some of the TF-CBT strategies Saiku's therapist, Dr. Hong, used to support his recovery? See slide 55 of Dr.Hertig's lecture materials if you need a reminder of TF-CBT treatment components.

I'm really not looking to take off points for the reflection assignments. I don't have specific rubrics for them, but if you do not seem to be engaging with the prompt or answering the questions, I may take off points. Each reflection assignment is worth
5 points.

For this assessment, you will report on the information that you collected in your interview, analyzing the interview data and identifying a past or current issue that would benefit from an interdisciplinary approach. This could be an issue that has not been addressed by an interdisciplinary approach or one that could benefit from improvements related to the interdisciplinary approach currently being used. You will discuss the interview strategy that you used to collect information. Your interview strategy should be supported by citations from the literature. Additionally, you will start laying the foundation for your Interdisciplinary Plan Proposal (Assessment 3) by researching potential change theories, leadership strategies, and collaboration approaches that could be relevant to issue you have identified. Please be certain to review the scoring guide to confirm specific required elements of this assessment. Note that there are differences between basic, proficient and distinguished scores.

As you complete the template, make sure you use APA format for in-text citations for the evidence and best practices that are informing your plan, as well as for the reference list at the end.

• Summarize an interview focused on past or current issues at a health care organization.
• Identify an issue from an interview for which an evidence-based interdisciplinary approach would be appropriate.
• Describe potential change theories and leadership strategies that could inform an interdisciplinary solution to an organizational issue.
• Describe collaboration approaches from the literature that could facilitate establishing or improving an interdisciplinary team to address an organizational issue.
• Communicate with writing that is clear, logically organized, and professional, with correct grammar and spelling, and using current APA style.
• Number of references: Cite a minimum of 3 sources of scholarly or professional evidence that support your central ideas. Resources should be no more than 5 years old.
• APA formatting: Make sure that in-text citations and reference list follow current APA style.
