Health Assessment

Arrange an appropriate time and setting with your volunteer “patient” to perform a skin, hair, and nails examination.

Download and review the Skin, Hair, and Nails Student Checklist and Key Points, provided in this week’s Learning Resources, and review the Seidel’s Guide to Physical Examination online media.

Perform the skin, hair, and nails examination, covering all of the areas listed in the checklist.

  • Skin: Start by observing the color, texture, moisture, temperature, and any lesions. Use a good light source. Ask the patient about any changes, itching, or pain.
  • Hair: Look at the color, distribution, and texture. Ask about any changes, loss, or scalp problems.
  • Nails: Observe the color, shape, and condition of the nails. Look for any abnormalities like ridges, spots, or discoloration. Ask about any changes or problems.

Remember to always explain what you’re doing to the patient and ensure they’re comfortable throughout the examination.

Nursing Assignment

Bioethical
 Decision
 Making
 Model

1. Define
 the
 dilemma:
 Use
 your
 own
 words
 to
 describe
 the
 problem.
 State
 it
 in
 a
 way

that
 others
 can
 quickly
 understand
 your
 dilemma.
 Review

2. Identify
 the
 medical
 facts:
 Describe
 the
 facts
 that
 are
 relevant
 to
 the
 dilemma.

3. Remember
 that
 the
 diagnosis
 and
 prognosis
 are
 medical
 facts.

4. Identify
 the
 non-‐medical
 facts
 (patient
 and
 family,
 external
 influences):

a. Patient
 and
 family
 facts
 such
 as
 culture,
 religion,
 social,
 economic,
 the

existence
 of
 an
 Advance
 Healthcare
 Directive,
 verbal
 preferences
 made
 by

the
 patient,
 how
 the
 patient
 lived
 his/her
 life.

b. Those
 that
 you
 discuss
 should
 be
 relevant
 to
 the
 situation.

5. External
 influences
 include:
 organizational
 policies,
 federal
 and
 state
 laws,
 practice

acts,
 code
 of
 ethics.
 These
 should
 be
 relevant
 to
 the
 situation.

6. For
 both
 step
 2
 or
 3,
 separate
 the
 facts
 from
 the
 assumptions:
 Sometimes
 all

healthcare
 professionals
 allow
 assumptions
 to
 guide
 their
 decision-‐making.
 These

must
 be
 identified
 so
 that
 these
 assumptions
 do
 not
 interfere
 with
 the
 process.

7. Identify
 items
 that
 need
 clarification.
 Your
 paper
 should
 identify
 facts
 that
 you
 need

to
 clarify.
 When
 initially
 discussing
 an
 ethical
 situation,
 it
 is
 not
 unusual
 to
 not
 have

all
 of
 the
 answers.

8. Identify
 the
 decision
 makers:
 Is
 the
 patient
 an
 adult
 competent
 to
 make
 their
 own

choices?
 Is
 the
 patient
 a
 child
 who
 is
 old
 enough
 to
 have
 a
 say
 in
 the
 decision.
 If
 the

patient
 cannot
 make
 their
 own
 decision,
 who
 is
 the
 decision
 maker?
 How
 was
 this

person
 selected?

9. Review
 the
 underlying
 ethical
 principles:
 Review
 which
 ones
 and
 why
 they
 apply
 t

this
 particular
 case:
 beneficence,
 nonmaleficience,
 veracity,
 fidelity,
 autonomy
 and

justice.

10. Define
 alternatives:
 One-‐Way
 to
 proceed
 may
 be
 apparent
 at
 this
 point.
 However,

sometimes
 there
 are
 different
 choices.

 They
 should
 be
 addressed
 identifying
 the

benefits
 and
 burdens
 for
 doing
 one
 thing
 versus
 the
 other.

11. Follow-‐up:

 Define
 the
 process
 to
 be
 used
 with
 the
 chosen
 alternative.

Advanced Nursing Inquiry and Evidence Based Practice

Identify 5 research articles and write a brief summary of the ethical approaches used in the research studies

Reflection week 7

Reflection: write 1-2 paragraphs reflecting on your learning for the week. Guiding questions are provided or you may write about what you felt was most significant to you for the week.

You will need to post your reflection here before you are able to see other students’ posts.

  • Nearly every student in the course was able to think of a time when they had personally been impacted by or had observed incivility in the workplace. In what ways does this impact your view of the nursing profession?
  • If those outside of healthcare were aware, how might it impact their view?
  • As you move towards your role as an NP, what will you do to support healthful work environments for yourself and your colleagues?
  • Think about your personal use of social media. Are there changes you would like to make in how you use social media?
  • What self-care practices do you engage in to promote personal holistic health?

MY NUMBER ASSIGNED WAS 1 WHICH IS: Level of Consciousness – Stuporous, Obtunded, Coma, Lethargy, etc.

Initial post:Each student will be assigned a number randomly.  Whatever your number is, select the corresponding topic below, then post a minimum of 5 bullet points about the topic.Your bullet points should address key components of the topic, such as what, how, who, & why.  This information should not be basic things you learned in Med/Surg, but rather advanced critical care based. Think about this as a group effort to create a study guide. Use ONLY your textbook, but do not cut & paste from the book. Then create, find, or borrow a test style question about your topic & post at the bottom of your bullet points. The format needs to be multiple choice or select all that apply. Think NCLEX style. Each week include a paragraph with the results from one of your weekly interviews.Discussion post assignments are worth 20 points each as follows:

  • 5 points for the quality of your bullet points.
  • 5 points for the quality of your question.
  • 5 points for answering the question of a peer as your response.
  • 5 points for the quality of your rationale.
    • Quality is defined as thorough and thoughtful while demonstrating professional level knowledge of the topic.
