The Doctor of Nursing Practice (DNP) degree has only been available to nursing scholars since 2004.  It was deemed necessary by the American Association of Colleges of Nursing (AACN) because “changing demands of this nation’s complex healthcare environment require the highest level of scientific knowledge and practice expertise to assure quality patient outcomes” (American Association of Colleges of Nursing, 2023).  Because the degree is relatively new, the significance of the DNP is not yet widely understood.  A recent study determined that “DNP-prepared nurses typically function as APRNs in clinical care or as health care system leaders, while there is a low number of DNPs in clinical practice settings” (Beeber et al., 2019).  That being said, there is a great opportunity here for the DNP-prepared nurses of the future.  Tussing (2018) identified that there is “great potential for innovation around new care delivery models, interdisciplinary projects, and community involvement for a healthier society” if healthcare leaders consider the potential of redesigning or creating new roles for the DNP-prepared nurse.

Expectations of Doctor of Nursing Practice versus Doctor of Philosophy in Nursing

            The DNP degree is founded in nursing practice whereas a Ph.D. in nursing is rooted in research and teaching.  I chose to pursue the DNP because I believe it will allow me to effect social change on a larger scale and I enjoy the clinical side of nursing.  As a DNP-prepared nurse, I will “hold a broad foundation of knowledge from nursing, ethics, psychosocial and biophysical sciences, as well as from organizational and change theories” (Hartjes et al., 2019).  My professional goal is to serve as a Healthcare organization’s Chief Nursing Officer (CNO).  Nurses who hold a Ph.D. tend to pursue research opportunities or serve as professors at Universities.  Obtaining my DNP aligned with my professional goal of becoming a CNO.  Additionally, Walden University offers an executive nursing track which will set me up for success.

                                        Addressing a Gap in Practice

            As a nurse executive, it will be my role to identify and address gaps in practice.  At my current organization, one gap I will address is the workflow of admitting patients.  I work at an inpatient psychiatric hospital.  The current practice is that patients arrive to the hospital via ambulance and are rolled directly onto the unit.  There is minimal communication between the intake department, which accepts the patient, and the nurses who receive the patient on the floor.  I aim to create an admissions unit that will house the intake department.  This will help facilitate communication and create a better experience for the patient.

My thoughts on trends in nursing have shifted a bit since week 1. A topic I addressed was nurse-to-patient ratios and how they are incompatible with providing the best care to patients. Although that issue still proves to be a problem, I feel more informed on the causes and effects of this trend. I’ve learned about legislative and environmental factors that come into play. Funding for institutions is limited and in order to address this issue properly, we must be informed and have background knowledge of the people we are appointing who make the decisions to allocate health care funds. Also, being more inclusive when giving positions in higher departments would be helpful. Nurses should have access to take on more leadership roles to be able to be involved in decision making. Nurses have first hand knowledge on what is needed on the floor. They understand the resources needed and they also can advocate for nurses who come from different backgrounds or have different levels of expertise. All in all, I have a better understanding of why this problem exists and also the ways in which we could actively rectify the situation.

A practice doctorate in nursing is a Doctor in Nursing Practice (DNP). Along the healthcare continuum, the DNP degree prepares nurses for nursing leadership and administration jobs in both clinical and non-clinical settings. According to Graves et al. (2021), a nurse who has earned a practice doctorate engages in advanced clinical practice and offers advanced levels of direct patient care. They use their specific knowledge and abilities to evaluate, identify, and treat complex health disorders. Nurses with practice doctorates are critical to leadership and collaboration in healthcare teams and systems. They work with physicians and other healthcare professionals to create and carry out care plans, enhance patient outcomes, and guarantee coordinated, all-comprehensive treatment.

Additionally, practicing nurses with doctoral degrees frequently work as educators and mentors, communicating their knowledge and experience into nursing students, other medical professionals, and colleagues of DNP nurses, who often fund one of their ideas and conduct research. Doctorly educated nurses must critically assess and apply theoretical frameworks to direct their practice and progress in nursing science. Although nurses with other degrees, such as the Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) certainly contribute significantly to nursing practice, it is crucial to remember that doctoral degrees typically come with higher expectations for research, leadership, theoretical comprehension, and planning. DNP graduates are expected to exhibit outstanding leadership and professional abilities, while BSN graduates are expected to deliver safe and competent patient care. Second, whereas DNP graduates are expected to have knowledge and skills in strategies for quality improvement and healthcare innovation, MSN graduates are expected to have advanced clinical abilities and expertise in their chosen specialty, particularly in advanced managerial roles.

By earning a DNP, nurses can increase their knowledge and proficiency in a particular area. My motivation stems from the information I can apply to handle complex mental health challenges and enhance patient outcomes. Assessment and improvement of the quality of healthcare delivery are specialties of DNP nurses. I may recognize, plan, and implement improvement opportunities to improve patient safety, lessen health inequities, and produce better results (Harrison et al., 2021). DNP-holding nurses are in the right position to influence public policy and improve the healthcare system. So that I may support equity, open access, and patient-centered care, I can uncover systemic gaps, make adjustments, and advocate for those changes.

Some of my experiences in addressing a gap in practice within an organization include aligning education with the practice environment. According to Fawaz et al. (2017), it will be necessary for nursing schools, organizations, practitioners, and students to all develop coping mechanisms for handling the sudden entry of new knowledge, ideas, and abilities. In addition, technology significantly impacts how we live our lives and how we practice, educate, manage, and do research. Process-based learning, which could delay the efficient implementation of my practice, is now underemphasized in nursing school.
