4. This assignment will be graded on the quality of the information, inclusion of one evidence-based scholarly resource, use of citations, use of Standard English grammar, and organization based on the required components (see the paper headings and content details in Part 1).
5. The length of the paper is to be 
between 1,000 and 1,500 words, excluding title page and reference list.
6. Create this assignment using Microsoft (MS) Word. You can tell that the document is saved as a MS Word document because it will end in “.docx.”
7. APA format is required in this assignment, explicitly for in-text citations and the reference list. Use 12-point Times New Roman font with 1-inch margins and double spacing. See the APA manual for details regarding proper citation. See resources under Course Resources, “Guidelines for Writing Professional Papers” for further clarification.

* Scholarly Sources: Only scholarly sources are acceptable for citation and reference in this course. These include peer-reviewed publications, government reports, or sources written by a professional or scholar in the field. The textbooks and lessons are

For the threaded discussions and reflection posts, reputable internet sources such as websites by government agencies (URL ends in .gov) and respected organizations (often ends in .org) can be counted as scholarly sources. The best outside scholarly source to use is a peer-reviewed nursing journal.  You are encouraged to use the Chamberlain library and search one of the available databases for a peer-reviewed journal article.  The following sources should not be used: Wikipedia, Wikis, or blogs.  These websites are not considered scholarly as anyone can add to these. In small groups, students will develop a visual communication project—an infographic—in which they use an image(s) to create a message that meaningfully informs or persuades an audience. The infographic should be like a poster you might see on the wall in a doctor’s office, containing a variety of types of information on a topic.  The topic does not need to be restricted to healthcare.  Be sure to follow APA throughout, treat all copyrighted images as research sources, and cite appropriately. Be sure to address all rubric criteria.

Required Format and Process

This assignment has two parts: 1) in groups, students will create an infographic (an image or a collection of images combined with text for the purpose of making a statement, illustrating a concept, or describing a process); students should decide early if they are aiming to inform or persuade, and research must inform the project; 2) the group will incorporate appropriate language to support the message to complete their infographic.

Example: To complete this assignment, each group  will: 1) choose a focus and goal (to either inform or persuade); 2) decide on the approach (illustrate process or make an impact); 3) select or create appropriate image(s); 4) conduct relevant research; 5) incorporate appropriate text to support the overall message in a well-designed infographic. Note: any online image must be cited just as text would (groups are free to create their own images for the infographic by using their own creative means, such as photography, drawing, etc.).

Care Coordination Presentation to Colleagues

INSTRUCTIONS-Develop a 20-minute video presentation for nursing colleagues highlighting the fundamental principles of care coordination. Create a detailed narrative script for your presentation, approximately 4-5 pages in length. This is a video presentation.

INTRODUCTION- This assessment provides an opportunity for you to educate your peers on the care coordination process. The assessment also requires you to address change management issues. 

PREPARATION– You are encouraged to complete the Managing Change activity. Completing course activities before submitting your first attempt has been shown to make the difference between basic and proficient assessment.

Scenario

Your nurse manager has been observing your effectiveness as a care coordinator and recognizes the importance of educating other staff nurses in care coordination. Consequently, she has asked you to develop a presentation for your colleagues on care coordination basics. By providing them with basic information about the care coordination process, you will assist them in taking on an expanded role in helping to manage the care coordination process and improve patient outcomes in your community care center.

To prepare for this assessment, identify key factors nurses must consider to effectively participate in the care coordination process.

You may also wish to:

· Review the assessment instructions and scoring guide to ensure you understand the work you will be asked to complete.

· Allow plenty of time to rehearse your presentation.

Recording Equipment Setup and Testing

Check that your recording equipment and software are working properly and that you know how to record and upload your presentation.
