This is a continuation of the health promotion program proposal, part one, which you submitted previously. 

Please approach this assignment as an opportunity to integrate instructor feedback from part I and expand on ideas adhering to the components of the MAP-IT strategy. Include necessary levels of detail you feel appropriate to assure stakeholder buy-in.

You have already completed the steps 1-4. 
Do not resubmit part 1. Make sure you revise this initial submission according to your instructor’s comments. 

To assist in maintaining harmony between Part I and 2 here you have a reminder of the previous paper outline:

· 1. Describe the health problem. Using data and statistics support your claim that the issue you selected is a problem. What specifically will you address in your proposed health promotion program? Be sure your proposed outcome is realistic and measurable. 

· 2. Describe the vulnerable population and setting. What are the risk factors that make this a vulnerable population? Use evidence to support the risk factors you have identified. 

· 3. Provide a review of literature from scholarly journals of evidence-based interventions that address the problem. After completing a library search related to effective interventions for your chosen health promotion activity, you will write a review that evaluates the strengths and weaknesses of all the sources you have found. You might consult research texts for information on how to write a review of the literature found in your search. 

· 4. Select an appropriate health promotion/disease prevention theoretical framework or conceptual model that would best serve as the framework guiding the proposal. Provide rationale for your selection which includes discussion of the concepts of the selected model

· 6. Thoroughly describe the intended outcomes. Describe the outcomes in detail concurrent with the SMART goal approach. 
The SMART goal statement should be no more than one sentence (1 paragraph). 

· 7. Provide a detailed plan for evaluation for each outcome. (1 paragraph). 

· 8. Thoroughly describe possible barriers/challenges to implementing the proposed project as well as strategies to address these barriers/challenges. (1 paragraph). 

· Finish the paper with a conclusion paragraph (1 paragraph) without typing the word “conclusion” before the paragraph.

  For this discussion board, I chose to appraise the role and responsibilities of the nurse and members of an interdisciplinary team across multiple healthcare settings in recognition, assessment, and interventions for the management of acute and chronic diseases. The role of the nurse is vital to patient care of acute and chronic diseases in many different ways. The nurse's role in all patient care is to educate the patient, advocate for adequate and ethical care, and be a middleman between the patient and the rest of the patient's care team. The nurses are the ones at the bedside assessing the patient and getting to know them. They then take this vital information to the other members of the care team like case management, specialists, or surgeons to provide adequate and holistic care. Without this information from the bedside, the patient wouldn't be able to be taken care of. The other members are vital to the patient's care by providing specific services that the nurse cannot like surgical intervention or certain medication. This class has shown me through each week's case study how important the interdisciplinary team is to the patient's holistic care of acute and chronic conditions. Going further in my practice I will make it a point to advocate for consulting the appropriate teams for adequate and quality care.

In my future nursing practice, I have identified “Evidence-Based Practice” and “Teamwork and Collaboration” as the two Massachusetts Nurse of the Future Nursing Core Competencies that will have a significant impact. By utilizing evidence-based practice, I can ensure that my nursing interventions are backed by current studies and clinical data. This approach allows me to critically evaluate data and incorporate it into my decision-making process, ultimately improving the standard of care I provide. Additionally, evidence-based practice promotes ongoing learning and development, enabling me to stay up-to-date with the latest developments and make decisions that best serve my patients' well-being. Teamwork and Collaboration are equally crucial, as they reflect the interconnected nature of healthcare delivery. Effective teamwork ensures seamless patient-centered care, which is essential in nursing practice, given the need to work alongside various healthcare professionals. Collaborating with diverse team members enhances my ability to gather varied perspectives, share knowledge, and collectively address complex patient needs. This competency fosters open communication, mutual respect, and a shared commitment to achieving optimal patient outcomes (Masters, 2021).

The intersections between both competencies are evident, as evidence-based practice often requires collaboration among interdisciplinary teams to implement new findings and collectively improve patient care. Person-centered care promotes personalized, comprehensive care that takes into account each patient's unique requirements, preferences, and experiences. By focusing on the person's family, societal, and cultural circumstances, nurses can deliver just, respectful, and evidence-based care. Patient-centered care is the foundation of nursing practice because it strengthens nurse-patient relationships, encourages shared decision-making, and enables patients to participate in their treatment (AACN, 2021).

In my practice, person-centered care will improve patient experience and health outcomes. I can personalize my care plans to my patients' requirements by learning and respecting their diverse backgrounds, attitudes, and beliefs. This strategy targets physical, emotional, psychological, and social well-being. I also believe in healthcare team communication, empathy, and teamwork, which person-centered care supports (Masters, 2021b).
