In this Assignment, you will identify an issue or opportunity for change within your healthcare organization and propose an idea for a change in practice supported by an EBP approach.

To Prepare:

  • Reflect on the four peer-reviewed articles you critically appraised in Module 4, related to your clinical topic of interest and PICOT (attached below)
  • Reflect on your current healthcare organization (Behavioral health hospital-children services/adults) and think about potential opportunities for evidence-based change, using your topic of interest and PICOT as the basis for your reflection (attached below)
  • Consider the best method of disseminating the results of your presentation to an audience.

Part 4: Recommending an Evidence-Based Practice Change

Create an 8- to 9-slide PowerPoint presentation in which you do the following:

  • Briefly describe your healthcare organization (Behavioral health hospital), including its culture and readiness for change. (You may opt to keep various elements of this anonymous, such as your company name.)
  • Describe the current problem or opportunity for change (psychiatric related). Include in this description the circumstances surrounding the need for change, the scope of the issue, the stakeholders involved, and the risks associated with change implementation in general.
  • Propose an evidence-based idea for a change in practice using an EBP approach to decision making. Note that you may find further research needs to be conducted if sufficient evidence is not discovered.
  • Describe your plan for knowledge transfer of this change, including knowledge creation, dissemination, and organizational adoption and implementation.
  • Explain how you would disseminate the results of your project to an audience. Provide a rationale for why you selected this dissemination strategy.
  • Describe the measurable outcomes you hope to achieve with the implementation of this evidence-based change.
  • Be sure to provide APA citations of the supporting evidence-based peer reviewed articles you selected to support your thinking.
  • Add a lessons learned section that includes the following:
    • A summary of the critical appraisal of the peer-reviewed articles you previously submitted (attached below)
    • An explanation about what you learned from completing the Evaluation Table within the Critical Appraisal Tool Worksheet Template (1-3 slides) (attached below)

 Esophageal Reflux Case Studies A 45-year-old woman complained of heartburn and frequent regurgitation of “sour” material into her mouth. Often while sleeping, she would be awakened by a severe cough. The results of her physical examination were negative. Although many patients with a hiatal hernia have no reflux, this patient’s symptoms of reflux necessitated esophageal function studies. She was found to have a hypotensive LES pressure along with severe acid reflux into her esophagus. The abnormal acid clearing and the positive Bernstein test result indicated esophagitis caused by severe reflux. The esophagitis was directly visualized during esophagoscopy. Her coughing and shortness of breath at night were caused by aspiration of gastric contents while sleeping. This was demonstrated by the gastric nuclear scan. When awake, she did not aspirate, as evident during the swallowing function study. The patient was prescribed esomeprazole (Nexium). She was told to avoid the use of tobacco and caffeine. Her diet was limited to small, frequent, bland feedings. She was instructed to sleep with the head of her bed elevated at night. Because she had only minimal relief of her symptoms after 6 weeks of medical management, she underwent a laparoscopic surgical antireflux procedure. She had no further symptoms. Chapter 1: The History of Healthcare

Hopefully you have seen how advances in science and technology have shaped and generated careers in the healthcare field. For this assignment please think about the career you are planning to pursue (this may change by the end of the course but that’s ok – just base your answer on what you are thinking of pursuing as of now). Write at least a paragraph (4-6 sentences makes up a paragraph; no more then 2 paragraphs total) describing at least two historical advancements or individuals from history (not family members) that you believe have been most influential in shaping your potential career. For example, if you are going to be a microbiologist in a hospital laboratory you may choose Dr. Koch’s postulates and the microscope as two of the major breakthroughs that influenced your field/career. Depending upon the degree of your familiarity with your intended career, these choices may take a bit of outside research. The advancements or individuals may be ones mentioned in the lecture or something or someone entirely different. In any case, you should be certain they are clearly relevant to your intended profession.

Be sure to include information on who/what the discovery/advance was and when it occurred/when the person lived or made their mark on the field. It should be clear from reading your post why this person or advance is important to your future career.

 Last week you provided an overview of each nurse theorist/and or nursing theory that you have chosen.
This week, please expand upon your initial post by describing a situation from your experiences in nursing in which you would apply your particular nursing theories.
To be clear, you will describe a situation in detail and then apply each of your nursing theory(ies) to that situation separately.
Be sure to pick and example that clearly illustrates the strength of each nursing theory in application to either nursing practice, nursing research, nursing education, or nursing administration.
Describe why the classification of each theory is appropriate for the situation you have described.
Compare and contrast the the usefulness of each of your theories to this particular situation.

Be sure to use appropriate APA citation and when responding to your peers, begin with addressing them personally and end your response posting with your name.
