Modern psychopharmacology is largely the story of chemical neurotransmission. One must be fluent in the language and principles of chemical neurotransmission to:

  • Understand the actions of drugs on the brain.
  • Grasp the impact of diseases upon the central nervous system.
  • Interpret the behavioral consequences of psychiatric medicines.

After studying Module 2: Lecture Materials & Resources, discuss the following:

  • Choose an FDA-approved medication currently used in psychiatry.
  • Explain the concept of that drug’s half-life.  
  • How long would it take for that drug to reach a steady state?  
  • How frequently should the medication be dosed based on the half-life? 
  • Use Epocrates.com as a reference for this assignment.

LEGAL AND ETHICAL ISSUES RELATED TO PSYCHIATRIC EMERGENCIES

The diagnosis of psychiatric emergencies can include a wide range of problems—from serious drug reactions to abuse and suicidal ideation/behaviors. Regardless of care setting, the PMHNP must know how to address emergencies, coordinate care with other members of the health care team and law enforcement officials (when indicated), and effectively communicate with family members who are often overwhelmed in emergency situations. In their role, PMHNPs can ensure a smooth transition from emergency mental health care to follow-up care, and also bridge the physical–mental health divide in healthcare.

In this week’s Assignment, you explore legal and ethical issues surrounding psychiatric emergencies, and identify evidence-based suicide and violence risk assessments.

THE ASSIGNMENT

In 2–3 pages, address the following:

· Explain your state laws for involuntary psychiatric holds for child and adult psychiatric emergencies. Include who can hold a patient and for how long, who can release the emergency hold, and who can pick up the patient after a hold is released.

· Explain the differences among emergency hospitalization for evaluation/psychiatric hold, inpatient commitment, and outpatient commitment in your state.

· Explain the difference between capacity and competency in mental health contexts.

· Select one of the following topics, and explain one legal issue and one ethical issue related to this topic that may apply within the context of treating psychiatric emergencies: patient autonomy, EMTALA, confidentiality, HIPAA privacy rule, HIPAA security rule, protected information, legal gun ownership, career obstacles (security clearances/background checks), and payer source.

· Identify one evidence-based suicide risk assessment that you could use to screen patients.

Informed Consent and Cultural Competence

Case Study:Dr. Emily Clark, a seasoned cardiologist at Mercy General Hospital, is presented with a challenging case involving Mr. Rajan Patel, a 58-year-old man who recently immigrated from India. Mr. Patel has been diagnosed with a significant heart condition that requires a surgical procedure. While the surgery has a high success rate, like all medical procedures, it comes with its own set of risks and benefits. Mr. Patel, whose primary language is Gujarati, has limited proficiency in English. He nods and smiles politely during consultations, giving Dr. Clark the impression that he understands. However, when asked to explain the procedure back to her, it becomes evident that he does not fully grasp the intricacies of the surgery or its potential complications. Dr. Clark recognizes the importance of informed consent and wants to ensure that Mr. Patel truly understands the procedure, its risks, and its benefits. She believes in the ethical principle of autonomy, which dictates that every patient has the right to make decisions about their own body and health. However, she also acknowledges the principle of beneficence, which compels her to act in the best interest of her patient.

The organization employs 3,000 people and provides extensive customer service training before employees are allowed to interact with customers.

Purpose

Often, after a class is done for the week, we drop it and move on. The goal of reflection assignments is to encourage you to reflect on what we did in class that week, after the class to consolidate your learning. There will be specific prompts for you to answer; sometimes this is a reflective assignment, and other times it is more of an application assignment (e.g., given a case example and having to apply something from class). The hope is that this will help you to use what you have learned to ensure concepts “stick” or to think critically about your response to what was learned.

Reflection #6

Sometimes it is helpful to provide information about the brain and neurobiology during psychoeducation with clients. Some clients are more or less interested in this, but for those who are interested, it can often be quite validating to know there may be physiological differences they are experiencing in response to stress.

Imagine that you wanted to provide psychoeducation to your client about some aspects of neurobiology as it relates to PTSD. Write out what you might say: (1) to an adult, and (2) to a child. If you would accompany visual images with this, you do not need to draw them, but could describe what they would include. Be sure to provide a bridge to what this information tells us about how we might work with them or what we might need to do to help them.

They would not need to know the level of detail we have covered, of course, so do not feel a need to explain everything!
